1. Introduction

Welcome to Believe Supported Living. These Terms and Conditions (“Terms”) govern your relationship with Believe Supported Living Ltd (“the Company,” “we,” “us,” or “our”). By engaging our services, you agree to comply with and be bound by these Terms.

2. Services Provided

Believe Supported Living provides a range of services including but not limited to personal care, daily living assistance, companionship, and healthcare coordination for elderly clients. The specific services to be provided will be outlined in an individualized care plan, which will be agreed upon with the client and/or their representative.

3. Care Plans

3.1 Assessment: Before the commencement of services, we will conduct an assessment to determine the needs and preferences of the client.

3.2 Care Plan Agreement: A care plan will be developed based on the assessment, outlining the services to be provided, the schedule of care, and any specific requirements. The care plan will be reviewed regularly and may be updated as needed with the consent of the client and/or their representative.

4. Client Rights and Responsibilities

4.1 Respect and Dignity: Clients have the right to be treated with respect and dignity at all times. They have the right to privacy, confidentiality, and autonomy over their care to the extent possible.

4.2 Involvement in Care: Clients have the right to be involved in decisions regarding their care and to have their preferences and needs taken into account.

4.3 Providing Accurate Information: Clients and their representatives are responsible for providing accurate and complete information regarding the client’s health, care needs, and any other relevant details.

5. Fees and Payment

5.1 Service Fees: The fees for services provided by Believe Supported Living will be outlined in the care plan agreement. Fees are based on the type and level of care required.

5.2 Payment Terms: Payments are due on the terms outlined in the care plan agreement. We reserve the right to charge interest on overdue payments under applicable laws.

5.3 Changes to Fees: Any changes to the fee structure will be communicated to the client and/or their representative with at least 30 days’ notice.

6. Cancellation and Termination

6.1 Cancellation by Client: Clients may cancel services by providing a minimum of 7 days’ written notice. Failure to provide sufficient notice may result in a cancellation fee equivalent to one week of service.

6.2 Termination by the Company: Believe Supported Living reserves the right to terminate services if the client fails to meet the agreed payment terms if the care needs of the client exceed what can be safely provided by our team, or if there is any breach of these Terms. A minimum of 14 days notice will be provided, except in cases of emergency or where the safety of the client or staff is at risk.

7. Liability and Insurance

7.1 Limitation of Liability: Believe Supported Living will provide services with reasonable care and skill. However, to the extent permitted by law, we are not liable for any loss or damage arising directly or indirectly from our services, unless such loss or damage is caused by our gross negligence or willful misconduct.

7.2 Insurance: We maintain appropriate insurance coverage for our services, including public liability insurance. Clients are encouraged to obtain their own insurance for personal property or specific needs not covered by our insurance.

8. Confidentiality

All personal information collected by Believe Supported Living is treated as confidential and will only be used to provide care services. We comply with all applicable data protection laws and regulations. Information will not be disclosed to third parties without the consent of the client or their representative, except where required by law.

9. Complaints and Feedback

9.1 Complaints: We are committed to providing high-quality care. If you have any concerns or complaints about our services, please contact us immediately. We have a formal complaints procedure, which is available upon request.

9.2 Feedback: We welcome feedback to improve our services. Clients and their representatives are encouraged to share their experiences with us.

10. Amendments to Terms

Believe Supported Living reserves the right to amend these Terms at any time. Any changes will be communicated to the client and/or their representative with at least 30 days’ notice. Continued use of our services following such notice constitutes acceptance of the revised Terms.

11. Governing Law

These Terms and any disputes arising out of or in connection with them shall be governed by and construed under the laws of the jurisdiction in which Believe Supported Living operates.
